This page contains important information about the procedures that must be followed if your child needs asthma medication during the school day. New Jersey law requires that each student authorized to use asthma medication pursuant to NJSA 18A:40-12.3, or a nebulizer, have an asthma treatment plan prepared by the student's physician. Therefore, please download the "Asthma Treatment Plan" form and bring to your child's physician, and then return the completed form (or a copy of it) to the school nurse.

Pursuant to the law, The Lenape Regional High School District shall permit students who require asthma medication to carry an inhaler at all times and self-administer prescribed asthma  medication  provided  that the pupil does not endanger self or others through misuse. In order for your child to self-administer medication, the following conditions must be met:

  1. The physician must certify that your child is capable and has been instructed in the proper method of self-administering of the non-nebuilized inhaled mediations indicated on the Pediatric/Adult Asthma Coalition of New Jersey (PACNJ) "Asthma Treatment Plan". By checking the box in the lower left hand corner of the "Asthma Treatment Plan", signing and stamping the plan with his/her signature your physician will have completed this
     
  2. The parent/guardian must sign the reverse side of the PACNJ "Asthma Treatment Plan" in order to provide the school nurse with your written authorization of your child to self-administer

Please be advised the Lenape Regional High SchooI District and its employees or agents shall incur no liability as a result of any injury arising from the self-administration of medication by your child and that you must agree by completing the consent form found under the Asthma Forms tab on the left side of this page, to indemnify and hold harmless the school district and its employees and agents against any claims arising out of the self-administration of medication by your child.

The permission for self-medication is effective for the school year for which it is granted and must be renewed for each subsequent school year upon fulfillment of the requirements listed above.

You are encouraged to provide the school nurse with a nebulizer to be used only for your child.  However, there is one nebulizer in each nurse's office for the care of students with asthma.
